3. Intake form and service providers
The MaiCareNow intake form is provided through Tally. When you submit the form, information is processed through Tally’s systems and made available to MaiCareNow. Tally states that form data is encrypted in transit and at rest and stored in Europe.

We may also use service providers for website hosting, email, document storage, communications, security, and technical support. These providers may process information only as needed to perform services for MaiCareNow or as required by law.
4. Health-information notice
Information you submit may be sensitive. MaiCareNow is not representing that this website or the current intake form is designed for the transmission of protected health information under HIPAA.
Submit only the minimum information needed to describe the situation. Detailed medical records, clinical documents, and highly sensitive identifiers should be shared only through a method that MaiCareNow has specifically approved for that purpose.
5. When information may be shared
